What is premature ovarian failure?

Premature ovarian failure is a disorder that can be seen in women before the age of 40. These people may not experience menstruation at all, or if they did experience menstruation, their menstruation stopped before the age of 40. It should be kept in mind that not all women who stop menstruating before the age of 40 have premature menopause, and this diagnosis must be confirmed with hormonal tests.

What is the cause of premature ovarian failure?

In many cases, there may be a genetic cause for the premature cessation of ovarian function, which causes the loss of eggs by disrupting ovarian function. In some cases, there may be an immune disorder that leads to an abnormal response of the body to its own tissues. Sometimes inflammation in the ovary due to some infections such as mumps causes premature menopause. Also, in some cases, chemotherapy or radiation therapy causes premature menopause. Surgeries that lead to the removal of the ovaries, causing great damage to the ovaries, or laparoscopies that are performed along with “cauterization” of the ovaries, i.e. burning the ovaries, can also be other causes of premature menopause or premature ovarian failure but it should be noted that in 90% of cases, the causes of premature ovarian failure remain unknown.

Complications of premature ovarian failure

  • Infertility: the inability to conceive can be considered one of the most important complications of premature ovarian failure, although there is a possibility of pregnancy.
  • Heart diseases: early loss of estrogen caused by premature ovarian failure can increase the risk of heart diseases.
  • Osteoporosis: estrogen helps in the proper balance of bones. Women with premature ovarian failure have lower estrogen levels. Thus, they are at risk of bone damage and osteoporosis.
  • Anxiety and depression: the possibility of infertility and other complications caused by premature ovarian failure can lead to anxiety and depression.
  • Dementia: a decrease in the amount of estrogen caused by premature ovarian failure can lead to the possibility of dementia in some people.

Diagnosis of premature ovarian failure

Diagnosis of premature ovarian failure includes physical examination and review of the patient’s records. Some tests that are used to diagnose premature ovarian failure include:

  • Prolactin test

Treatment of premature ovarian failure

In these people, together with the relevant controls, estrogen tablets with progesterone are given, and sometimes this treatment leads to small monthly bleedings, but one should not be mistaken and think that with this work, the function of the ovaries has returned; Of course, in a small number of cases (about 5-10%), the ovary returns to normal activity for a short period of time and the possibility of fertility also arises. In most of these people, it is not possible to get pregnant naturally, but they can use a donated egg for fertilization. Patients with premature ovarian failure should walk for half an hour every day or do sports that make bones strong, use calcium-containing materials such as dairy products and enough vegetables in their diet. Taking calcium D drug and estrogen and progesterone drugs are recommended to prevent osteoporosis under the supervision of a doctor.
